Specialized care for cardiovascular patients

Heart Touch Journey is Alleo Health System’s patient and family centered clinical specialty program designed to help cardiac disease patients stay in their home setting and reduce uncomfortable and lengthy trips to the emergency room. Heart Touch Journey includes a specially-designed cardiac medication kit to keep in the home. Education and support are provided to the patient and caregivers regarding the medications and their usage, and guidance is offered about when to call hospice for assistance with pain management, additional support, or emergencies.

Benefits of Heart Touch Journey:

  • Cardiac comfort kit of medications in the home to manage pain and symptoms

  • Daily calls or visits if needed to reduce uncomfortable and lengthy trips to the emergency room

  • 24/7 access to an on-call nurse

  • Better quality of life at home through physical, emotional, and spiritual support

  • Reduced readmission rates

  • All hospice patients with a cardiac diagnosis are eligible for Heart Touch Journey.

  • There is no cost for the program under the hospice benefit. All related medications, supplies, and equipment are paid for under hospice.

  • Education and support are provided to patients, caregivers, and families regarding medications, and 24/7 support is available for questions.
