Specialized care for end-stage pulmonary patients

Clear Journey is Hospice of Chattanooga’s patient and family centered clinical specialty program designed to help pulmonary disease patients stay in their home and reduce emergency room visits. Clear Journey includes specially-designed pulmonary medication kit kept in the home. Education and support are provided to patients, caregivers, and families about the medications and their usage, and guidance is offered about when to call hospice for assistance with pain management, additional support, or emergencies.

  • Pulmonary comfort kit of medications in the home to manage symptoms such as difficulty breathing, anxiety, and pain

  • Daily calls or visits to reduce uncomfortable and lengthy trips to the emergency room

  • 24/7 access to an on-call nurse

  • Better quality of life at home through physical, emotional, and spiritual support

  • Reduced readmission rates

  • All hospice patients with a pulmonary disease diagnosis are eligible for Clear Journey.

  • There is no cost for the program under the hospice benefit. All related medications, supplies, and equipment are paid for under hospice.

  • Education and support are provided to patients, caregivers, and families regarding medications, and 24/7 support is available for questions.
