That focus led to the creation of the Heart Touch Journey program at Alleo Health System. This specialized program for hospice and palliative care patients living with a cardiac disease diagnosis provides treatment at home tailored to meet the specific needs of cardiac patients. 

The program provides 24/7 access to an on-call nurse or nurse practitioner, a cardiac medication kit, and a team of caregivers to help keep patients at home and avoid lengthy and uncomfortable trips to the emergency room. Education on the use of the medication kit is provided, and daily calls or visits from medical providers are offered according to a patient’s preferences. 

These features make it easier for individuals to avoid potentially exposing themselves to infection or disease, because they can receive care wherever they call home. The additional support also helps reduce instances of patient readmission to the hospital. 

The Heart Touch Journey program also provides education and support so that individuals will know how to handle potential emergencies or when to call hospice or palliative care for help. Support from social workers and spiritual care coordinators helps patients manage more than just physical well-being. This all-encompassing approach provides stability to families navigating the challenging circumstances of a serious illness.
